What makes a confined space 'permit-required'?
A confined space becomes permit-required if it contains (or could contain) a hazardous atmosphere, a material that could engulf an entrant, an internal configuration that could trap or asphyxiate, or any other recognized serious safety or health hazard.
Is a fire watch required for hot work?
A fire watch is required whenever hot work is done where more than a minor fire could develop, and it must be maintained during the work and for at least 30 minutes after it ends to catch smoldering fires.
Does lockout/tagout need machine-specific procedures?
Yes. Except for limited single-energy-source cases, employers must develop, document, and use energy-control procedures specific to each machine, covering the steps to shut down, isolate, lock out, and verify zero energy.
