Sulphur, Oklahoma - shop fatality · 2021

On September 29, 2021, a worker was heating a drill head while a heavily modified pipe wrench held it against rotation, the wrench handle braced under the raised right fork of a forklift. The 70.2-pound handle came loose from under the fork and struck him in the head, killing him. OSHA cited 1910.178(m)(5)(iii), which requires that when the operator is dismounted and within 25 feet of the truck, the load-engaging means be fully lowered, controls neutralized and brakes set, together with 1910.212(a)(1) machine guarding and 1910.242(a). The forklift here was not moving freight; it was used as a fixture.
